Allied Pain & Spine Institute is not a participating provider of the Covered Care/Health Exchange plans that are linked with Blue Cross and Blue Shield. We also do not accept Blue Shield EPO, MediCal and MediCaid. We do accept Blue Shield PPO and Blue Cross PPO, Blue Shield FEP and Blue Cross FEP as well as many other PPO insurance plans. We also accept MediCare PPO and MediCare HMO with authorization from your Primary Care Physician.

When scheduling your initial appointment, please be prepared to email/fax/scan your insurance cards/ID for authorization/verification of benefits. If being referred by an HMO Plan, please obtain and fax authorization prior to scheduling your initial appointment. Please be prepared to show your insurance card and ID at every appointment.
